In this needs assessment, identif one key health need, and planning and critically analysing an evidence-based health promotion intervention in accordance with current national and/or international health, social care and/or public health policy (framework, guidance and/or strategy) to address it.
Conduct a comprehensive and systematic, evidence-based health and social care needs assessment of your specialist practice area, considering wider determinants of health, using local, regional, national and/or international epidemiological and statistical data to critically examine the local community’s needs, and identify the current health and/or social care needs relevant to your scope of specialist practice.
Based on your community health and/or social care needs assessment, analyse one key public health, health and/or social care need (dependent upon your specialist scope of practice) and plan an evidence-based health and/or social care promotion intervention to address the specific needs of your identified local community and/or population.
Identify, critically discuss, analyse, and apply a key city, regional or national public health, health and/or social care policy, framework, strategy, guidance, plan, or equivalent document that outlines specific goals, targeted implementation measures and/or the allocation of financial and human resources relevant to your identified health and/or social care need. You should critically discuss and analyse the strengths and weaknesses of the identified key regional or national public health, health and/or social care policy, framework, strategy, guidance, plan, or equivalent document.
Critically examine the knowledge, understanding and application of the underpinning health promotion model/theory you will use to support the implementation of your intervention.
Critically reflect on how you will involve persons, users of service, and their carers, local communities and/or populations (dependent upon the nature of the identified key public health, health and/or social care need) in planning and the implementation of your intervention.
·Critically discuss the SMART outcome/s you want to achieve, how these outcomes would be measured and evaluated, so that you can identify how they will contribute to addressing current city, regional or national public health, health and/or social care priorities.
